Origin and Classification
4-Methylmethcathinone (mephedrone), 3,4-methylenedioxymethcathinone (methylone), and 3,4-methylenedioxyprovalerone (MDPV) are the most common synthetic cathinones, all originated from cathinone, an alkaloid with stimulant properties present in the leaves of Catha edulis, and known as khat (German, Fleckenstein, & Hanson, 2014; Valente, Guedes de Pinho, de Lourdes Bastos, Carvalho, & Carvalho, 2014)
Synthetic cathinones are heterogeneous with respect to mechanism of action. Butylone, ethylone, mephedrone, methylone, and naphyrone function in a manner similar to cocaine, serving as nonselective inhibitors of monoamine reuptake. Butylone, ethylone, mephedrone, and methylone cause serotonin release. Cathinone, methcathinone, and flephedrone act in a manner similar to amphetamines, preferentially causing the release of dopamine and inhibiting dopamine and norepinephrine reuptake (Simmler et al., 2013). Pyrovalerone and 3,4-methylenedioxypyrovalerone (MDPV) inhibit dopamine, serotonin, and norepinephrine transport (Schifano et al., 2016), but – unlike amphetamines – do not cause the release of monoamines (Simmler et al., 2013).
The main effects and risks of mephedrone include euphoria, alertness and feelings of affection towards the people around, as well as feelings of anxiety and paranoia. Mephedrone has physical effects: for example, on the cardiovascular system, with tachycardia and arterial constriction leading to cold, blue hands and feet. Excessive CNS stimulation may cause headaches, insomnia, agitation and hallucinations, as well as seizures, which may be fatal. Snorting mephedrone can damage the nose, causing nosebleeds. Other adverse effects include bruxism, nausea and vomiting, appetite suppression, sweating and changes in body temperature. Most of those adverse effects are like those of ecstasy and cocaine. Anecdotal reports suggest that heavy use can lead to paranoia, hallucinations and serious panic attacks.
